2009 REGISTRATION INFORMATION
FOR NEW OR RETURNING PARTICIPANTS
The following information will be required for you to register your dog for the dog park:
In the effort to stay green, the OLDA Renewal or Registration Agreement-Waiver is available below or when you come into the Recreation Activities Center in the WB Civic Center Complex to register. Owners must completely fill out the Agreement-Waiver of Liability and Indemnification Form and bring it and the following information in person.
2. Bring your paper Oakland County Dog License, not the tag, not a copy or fax to our office. County dog licenses are available the township treasurer’s office. When you provide our office proof of your 2009 Oakland County Dog License you will be issued a special West Bloomfield Parks and Recreation permit tag for your dog(s). This tag must be worn by the dog and be visible at all times.
3. Bring proof of up to date vaccinations, including Bordatella, at the time of registering. A copy will be made of the original, no faxed or copies will be accepted.
4. Bring your drivers license. A copy will be made of the original, no faxed or copies will be accepted. Renewals or new registrations will not be permitted if the County Dog License, Vaccination papers and drivers license do not match exactly.
5. Grandfathered, non-resident permit holders may renew their permit so long as all information remains the same. Any change in dog ownership or place of residence will preclude you from registration. No additional dogs or additional household members may be added from your previous permit.
6. Pay the fee of $20 resident. ($30 for grandfathered non-resident.) Non-residents may not apply as new registrations. You must be a West Bloomfield Township resident to register the first time for the Off Leash Dog Park.
